Insurance

I am a contracted provider with Optum/United Behavioral Health (which includes Oscar and Oxford Health Plans).

Rates

Those who have another insurance plan will be responsible for funding the entire cost of treatment. Please see “Treatment Consent” under Forms for my fee schedule. I do provide an itemized bill for those who wish to apply for reimbursement from their own PPO plans.

Payment

Cash and check are accepted as payment. Payment is due at the end of each session, unless otherwise specified.

Cancellation Policy

Appointments must be cancelled at least 48 hours in advance.
